INTERNATIONAL HEADACHE SOCIETY TRAVEL GRANTS

The IHS Trainees and Residents Sub-committee is pleased to offer 12 Research and Educational travel grants to attend the European Headache and Migraine Trust International Congress 2008. The deadline for applications is 1 July 2008 and applicants must be under 35 years of age. REGISTRATION FEES
Registration fees are quoted in UK pounds sterling, are inclusive of VAT and provide attendees with morning coffee on three days of the congress and lunch, afternoon tea on all four days of the congress and an invitation to the Welcome Reception.

STUDENT RATE*
PhD/Research students will be eligible for a reduced registration fee. Registration forms must be accompanied by a letter from the Head of Department or Supervisor, clearly stating the student’s current position.

ACCOMPANYING PERSONS
Accompanying persons will have full use of the symposium complex with morning coffee, lunch and afternoon tea during the symposium and an invitation to the Welcome Reception. They are not entitled to attend symposium sessions.

TEACHING COURSES
The teaching courses are offered free of charge to delegates attending the Congress. If you wish to only attend one of the teaching courses, the cost is £75.00 per course.

METHOD OF PAYMENT
Payment can be made either by sterling cheque (payable to EHMTIC Ltd), credit card (Visa, MasterCard or American Express) or debit card (Maestro or Visa Delta). Credit card payments are subject to an additional charge (Mastercard/Visa/Amex: 2.95%). Confirmation of registration, together with a receipt for payment, will be forwarded by the Secretariat. Places at the meeting cannot be reserved in the absence of a registration form and payment.

CONFIRMATION OF REGISTRATION
Written confirmation and a VAT receipt of payment will be sent to you.

CANCELLATIONS OF REGISTRATION FEES
Please advise the Secretariat in writing, should you need to cancel your place at the Congress. Cancellation of individual registration bookings received in writing by the Secretariat will be refunded after the Congress at the following rates:
Before 1st August 2008 – 50% of registration fee
Between 1st – 21st August 2008 – 25% of registration fee
From 22nd August 2008 – We regret no refund can be made although name substitutions can be made at any time.

Participants and their partners are advised to take out their own travel insurance and to extend their personal policy to cover personal possessions: the Congress does not cover individuals against cancellations of bookings or theft of belongings.

In the event that the Congress is cancelled for any reason or any factor (whether or not within the control of The European Headache and Migraine Trust International Congress Migraine Trust or the Congress Secretariat), the amount of the Congress Fee shall be refunded. The total liability of The European Headache and Migraine Trust International Congress Migraine Trust and the Congress Secretariat shall be limited to that refund only and it shall not be liable for any other loss, cost or expense, howsoever arising.

The Secretariat will hold and process your personal details on our database. This information is necessary for the legitimate management of this congress, which may include it being available to any organising/sponsoring company. We may also use these details to provide you with information of other conferences or events. Please advise us should any of your details change. You may of course remove your details from our database by advising us in writing, at any time.
